Go to wellness360.com and join. Then log into your account with the ID and password they give you. At the top of the page click on 'My Medical Record' and click on 'Member ID card'. You will see where it allows you to print your card. When you click to print your card it will open up in adobe reader, at the bottom of the page there is a $5/$15 coupon and one for $30 w/ transferred prescription. Both expire 3/31/08
Save this PDF file to your computer to print coupons as needed!
